The assessment of adequacy is the first step in the evaluation of a thyroid fine-needle aspiratio... more The assessment of adequacy is the first step in the evaluation of a thyroid fine-needle aspiration (FNA) sample. A specimen with adequate cellularity is a key to the success of thyroid cytology because it assures minimal false-negative rate. The adequacy of a thyroid FNA is defined by both the quantity and quality of the cellular and colloid components. According to the Bethesda system for reporting thyroid cytopathology, a specimen is defined as non-diagnostic if it fails to meet the adequacy criteria (at least 6 groups × 10 benign-appearing, well-visualized follicular cells), with exceptions for thyroiditis, abundant colloid, or any degree of atypia. This chapter discusses cytologic criteria of adequacy along with some controversial or currently evolving topics, such as cyst fluid-only aspirates, adequacy in liquid-based preparations, and proposed modifications of the adequacy criteria. A clinically oriented part is devoted to non-diagnostic thyroid nodules and their management. The last section describes some practical hints and techniques to improve adequacy. The chapter is addressed primarily to cytopathologists; however, it would be useful for each member of the thyroid patient care team, including clinicians (FNA operator and physician in charge) and cytotechnologists.

A polymerase chain reaction (PCR) protocol for detecting IS6110 repetitive insertion sequence of ... more A polymerase chain reaction (PCR) protocol for detecting IS6110 repetitive insertion sequence of Mycobacterium tuberculosis (MTB) was tested on archival Papanicolaou (Pap)-stained fine needle aspirated (FNA) smears from 24 patients with cervical tuberculous lymphadenopathy and 30 negative controls. The protocol involved protease digestion or phenolchloroform extraction, and simple or nested PCR, with PCR amplification of human beta-globin gene for internal control of DNA quality. Sensitivity of 50% and specificity of 100% were obtained. Sensitivity in smears showing necrosis without granuloma was 70% (7/10), whereas it was 36% (5/14) in smears with presence of granuloma. On the other hand, sensitivity of 18% (4/22) was obtained using FNA acid-fast stain, 25% (1/4) for acid-fast stain in histological section, 50% (2/4) for culture, and 100% (8/8) for PCR of fresh specimens. PCR for MTB detection in Papanicolaou-stained slides is a practical and valuable method when no fresh specimen but only Pap-stained smear is available.

This lesion often presents as a spiculated lesion, sometimes with microcalcifications, on mammography which may mimic malignancy such as tubular carcinoma. We report a case of radial scar with clustered microcalcifications incidentally found in the screened mammogram.

Metastases to the breast are uncommon, accounting for 0.5% of breast tumors, and most of them are... more Metastases to the breast are uncommon, accounting for 0.5% of breast tumors, and most of them are originated from lymphoma, melanoma and carcinomas of various organs. Alveolar soft part sarcoma (ASPS) is a very rare neoplasm that is usually found in the lower extremities. Lungs are the common site of dissemination and may represent initial manifestation of disease. We report a clinically unsuspected case of ASPS presenting as a breast metastasis in a 25-year-old woman. The patient's medical history was notable for a thyroid cancer treated by surgery and radioiodine ablation 2 years ago. Core needle biopsy of slowly growing breast mass yielded polygonal cells with abundant eosinophilic cytoplasm arranged into solid pattern. Differential diagnosis between apocrine cell carcinoma, paraganglioma, granular cell tumor, neuroendocrine carcinoma, ASPS and metastatic hepatocellular and renal cell carcinoma was rendered by immunohistochemistry. Strong nuclear TFE3 immunoreactivity confirmed a diagnosis of ASPS. Retrospectively a primary tumor was found in the thigh. Most likely, ASPS and thyroid cancer in the patient were growing synchronously and independently.

The differential diagnosis between malignancy-related ascites (MRAs) and nonmalignant ascites (NM... more The differential diagnosis between malignancy-related ascites (MRAs) and nonmalignant ascites (NMAs) has remained an essential problem in clinical practice. Our purpose was to determine the diagnostic value of ascitic fluid telomerase activity in discriminating these two categories compared with that of cytological examination. Twenty-five MRAs and 47 NMAs as the control group were enrolled in our study. In the MRA group, telomerase activity was detected in 13 of 16 (81.3%) cases of peritoneal carcinomatosis and in 6 of 9 (66.7%) cases of hepatocellular carcinoma (HCC)-associated ascites. Contrasting that, cytological examination was positive in only 9 of 16 (56.3%) and 1 of 9 (11.1%) cases, respectively. In the NMA group, telomerase-positive ascitic fluid samples were found in 2 of 47 (4.3%) cases, all belonging to subgroups that contained large numbers of lymphocytes in the ascites. In our study, the telomerase activity and cytological examination exhibited a sensitivity of 76% and 40% and a specificity of 95.7% and 100%, respectively. Regarding subgroups of MRAs, the telomerase activity and cytological examination demonstrated a sensitivity of 81.3% and 56.3%, respectively, in peritoneal carcinomatosis and a sensitivity of 66.7% and 11.1%, respectively, in HCC-associated ascites. In conclusion, telomerase activity is a more sensitive marker than cytological examination for differentiating between MRAs and NMAs. It may also serve as a useful indicator for detecting early i.p. metastasis in HCC-associated ascites.

Health- Related Quality of Life (HRQOL) is an important evaluation of health outcome that has not been studied in Thai patients with ostomy. Objectives: We determined HRQOL of patients living with ostomy, correlated factors, and average cost of ostomy supplies per month. Methods: Cross-sectional study with questionnaires was carried out given to 107 patients with ostomy at King Chulalongkorn Memorial Hospital. Self-administered individual factors questionnaire, WHOQOL-BREFTHAI questionnaire, and ostomy appliances satisfaction questionnaire surveys were employed. Results: Response rate was 75.9%. Cronbach’s alpha of the WHOQOL-BREF-THAI questionnaire was 0.883. The mean of overall HRQOL scores was 82.5 (SD = 11.0) out of 130. It reveals that most participants had moderate HRQOL levels and were satisfied with ostomy appliances. Social relationships domain of WHOQOL-BREF-THAI had the highest number of participants who had poor HRQOL level. Significant factors related to HRQOL were gender, age groups, obesity, marital status, payment scheme, financial status, skin disorders, and underlying diseases. The mean cost of ostomy supplies per month per stoma was 1,770 Thai Baht (SD = 858.9). One-piece ostomy appliance system had lower cost with better HRQOL scores and better ostomy appliances satisfaction scores. However, the sample size was limited for the one-piece system. Conclusion: Most Thai patients living with ostomy have moderate quality of life, and they adjust well. The social relationships compromises are major issues that healthcare professionals should address with this group of patients. Ostomy supplies can be an important expense for patients.

Osseous choristoma represents the dense, mature, bony tissue in an abnormal location. We reported... more Osseous choristoma represents the dense, mature, bony tissue in an abnormal location. We reported three cases of lingual osseous choristoma, herein, a year after the report of eight cases in 1998. All three lesions were at or close to the foramen caecum. The lesions were smooth, round or lobulated and pedunculated in shape and stony hard in consistency. All of them were preoperatively diagnosed, based on the above unique clinical findings without any radiography. The lesions were simply excised without local recurrence.
