Microsatellite Instability of Colon adenocarcinomas in India comprises multiple molecular subtypes

The microsatellite stable category is more than four-fifths of global colon and rectal cancer cas... more The microsatellite stable category is more than four-fifths of global colon and rectal cancer cases (CRC). But many studies on the Indian population during the last two decades have shown that the microsatellite instable CRC was more than 30%. We have conducted a study to explore the possible pathogenesis of microsatellite instability in Indian CRC. In the preliminary investigation, we conducted a Nanostring Pan-Cancer pathway expression analysis of early-stage CRC tumor-normal pairs (n = 11, MSS = 5, MSI = 6). The differentially expressed genes associated with the tumor and correlated against microsatellite instability status were identified. Among them, AXIN2, ETV4, and RNF43 were tumor cell-specific signals showing important differential expression patterns between MSI and MSS groups. The expression of these genes was further validated in another set of microsatellite instable early-stage CRC tissues (n = 15) by qPCR. The expression fold-changes of these signals suggest a distinct subset of MSI subgroups of CRC in the Indian population. Further validation in a larger cohort could enable a better understanding of the pathogenic mechanism as well as clinical utility of this expression pattern.

e16065 Background: The molecular pathogenesis of colon adenocarcinoma (COAD) is attributed to lar... more e16065 Background: The molecular pathogenesis of colon adenocarcinoma (COAD) is attributed to large molecular changes such as Chromosomal Instability and high somatic copy number variation or defective DNA mismatch repair and consequent microsatellite instability (MSI). The prevalence of this type of cancer is increasing in the state of Kerala, India. A detailed study of the signaling pathways could lead to a better understanding of the central molecular pathological changes and potential biomarkers particular to this population. Methods: High throughput somatic expression analysis using Nanostring PanCancer pathway panel assay was performed. Differentially expressed (DE) genes were selected and KEGG pathway enrichment analysis was performed for assessment of canonical signaling pathways. Expression profiles were compared against the public database, Colon Adenocarcinoma cohort of the Cancer Genome Atlas (TCGA-COAD), using the Genome Expression Profiling Interactive Analysis (GEPIA). Protein-protein interaction network analysis was done using Cytoscape. DE genes were compared against immune cell infiltration in the TCGA-COAD from Tumor Immune Estimation Resource (TIMER) databases. Results: Nanostring assay was performed in 10 Tumor-Normal paired FFPE samples of stage II/III colon adenocarcinoma. Out of &amp;amp;amp;amp;amp;amp;amp;gt; 700 genes, significant difference in expression was found in 83 genes (FDR adjusted p -value &amp;amp;amp;amp;amp;amp;amp;lt; 0.01) with fold-change |fc(log2)| of at least one. Fold-change |fc(log2)| ≥ 2 was found in 19/83 genes. Network analysis clustered 13 out of 17 upregulated genes. Superimposition of the current data on TCGA study showed that among the three genes, four (MET, MCM2, ETV4 and MMP7), were common. DE Genes which were not significant in TCGA COAD study such as INHBA, COL1A1, COL11A1, COMP, SFRP4, SPP1, IL11, LIF, WT1 and DDIT4, were found to be significant in the current study. A few of the unique genes identified in the current study were found to have significant correlation with antigen presenting cells infiltration in the TCGA-COAD studies. Conclusions: Many DE genes from the study population were found to be different from previous large cohort studies in other population. This may suggest a different pathogenesis of COAD in this population, warranting a detailed study on the pathogenesis.

Serum proteins, serum iron and total iron binding capacity were estimated in 50 patients with ora... more Serum proteins, serum iron and total iron binding capacity were estimated in 50 patients with oral submucous fibrosis and 50 patients with oral leukoplakia. The values were compared with that of 50 age- and sex-matched controls. A significant depression in hemoglobin and serum iron was observed in both groups of patients, whereas total iron binding capacity showed significant change only in the oral submucous fibrosis patients. Serum protein values were significantly lower in all the patients. The role of iron deficiency anemia in the causation of this premalignant lesion is discussed.

In the preliminary studies, we conducted a Nanostring Pan-Cancer pathway analysis of early-stage CRC (n = 10, MSS = 5, MSI = 5) and normal tissues (n=7). We identified the differentially expressed genes associated with the tumor and correlated them against microsatellite instability status. Among them, AXIN2, ETV4, and RNF43 were tumor cellspecific signals that had a differential expression between MSI and MSS groups. When overlapped with the TCGA immune cell infiltration data, TIMER, these genes segregated to the tumor cells. Moreover, they were less associated with other significant genes in protein-protein interaction analysis by STRING. The expression of these genes was further validated in another set of early-stage microsatellite instable CRC (n = 15) by qPCR. The expression foldchanges of these signals suggest distinct subsets in the MSI subgroup of CRC in the Indian population.
